In descending order of difficulty:
- In the %ALLUSERSPROFILE%\Media Center Programs\ (C:\ProgramData\Media Center Programs\) folder create a shortcut to the application. By far the easiest method but limiting in terms of where it goes and what icon is shown.
- Create registry keys for the application. This is handled in several of the subkeys of [HKEY_LOCAL_MACHINE\SOFTWARE\Microsoft\Windows\CurrentVersion\Media Center\Extensibility]. You'd need to make an entry in Entry Points, Applications, and one under Categories for each location you want a link shown. This is a little more complicated but plenty of free tools to help you manage them. What I don't personally like about this is that the image you assign a tile is limited to a square png while the default MS ones can be rectangle so they don't look as good IMHO.
- Create a Discovery Package similar to MCESpotlight and Sports. For me this was the option I went with but it involves development. You have to implement different PackageMarkupQuickLink and meant to be used by 3rd party partners for media center applications so there has to be a place holder for the menu
