I am having the same problem. I emailed Avermedia with the problem. This is the response I got:

The current *.29 drivers are the most stable and is
Microsoft certified.
Since there are so many configurations of hardware for
Vista X64, the stability can be compromised with some particular set-ups.
If haven't done so, you also need the system board
drivers from the motherboard manufacture specific to 64 bit OS.
The motherboard needs specific drivers from the motherboard
manufacture in order run devices on the PCIe bus. You might be running generic drivers from
Microsoft that will still report running properly, but isn't."
Of course I have all latest drivers on my PC. I had the error prior to a clean install of Vista with SP1. For a number of weeks it was fine, but now every time the PC comes out of sleep and open VMC and go to live to TV the APC_Index_Mismatch error occurs.
I don't know what the solution could possibly be.
My setup: AMD Athlon 4800 X2, Foxconn 6150BK8MC-KRSH, 4GB GSkill DDR 400, 74Gb WD Raptor, ATI x1900GT. Vista x64Bit Ultimate SP1.
Is there anybody who can help with this issue?
